Treatment Guidelines


UVB For PSORIASIS

24hrs-MED

  1. Initial dose: - 70% of MED.
  2. Increment: - Increase up to 20% each visit

  3. - At the start we can increase up to 40% of MED.

    - If No erythema on normal skin -> Increase by 20% of MED.

    - If starting MED <100 mJ/cm2 -> Increase by ³ 10 mJ/cm2).

  4. No of Sittings: 3-4 sitting / week.
  5. Followup: - 60-70% improvement -> Increase by 5-10% of MED.
  6. - ³ 80%-> Hold last dose

    - ³ 90% -> Stop.

  • Missed sittings:
  • - 1-2 missed sittings-> previous dose

    - 3 missed sittings-> dose prior to last dose

    - ³ 4 missed sittings-> repeat phototest

  • Adverse Effects:
  • - Mild erythema: same previous dose.

    - Moderate erythema: same previous dose, next sitting -> Increase by 10% thereafter.

    - Severe erythema: No treatment sitting (steroids, NSAIDs) & repeat phototest.

    - Itch -> emollient + antihistamine.

    - Facial erythema -> sunscreen.

  • Remarks:
  • limit the use of topical steroids + encourage frequent use of emollient before ttt by applying a very thin layer of emollient.
